Free Consultation:

A brief phone consultation is offered in order to get basic background information, answer questions, help determine if an evaluation is appropriate, and if so, schedule a time for the Formal Initial Intake Interview.

Formal Intake Interview:

A non-refundable deposit of $150 is required to schedule a Formal Intake Interview with the parent and/or client, and generally lasts between 60-90 minutes. During this session, detailed background information will be collected and discussed. The specific areas of evaluation will be determined during this session. Certain evaluation packages like IQ testing or Academic Testing for non-diagnostic reasons or 5k Readiness testing does not require a Formal Intake Interview.

Evaluations:

The total cost of the evaluation varies depending on the type of evaluation that is required to answer the referral questions. The type of evaluation is determined during the Formal Intake Interview, as evaluations are uniquely tailored for each individual. Rates are calculated based on the amount of time needed to complete a review of records, consultation, face-to-face testing (typically 2-6 hours) and parent feedback session (typically 60-90 mins).

A good faith estimate for the evaluation will be provided after the Formal Intake Interview, however rates generally range as follows:

• Kindergarten Readiness, IQ Testing, Academic Testing, or Giftedness Evaluations: $350 – $750

• Learning Disability, Developmental Delay, Intellectual Disability, ADHD, Autism, or Full Psycho-Educational Evaluations: $750 – $1,400+

Additional payment information:

If an evaluation is agreed upon, 50% of the balance is due in order to schedule the first testing session, and the remaining balance is due before results and feedback will be provided. Full payment can be made at any time. There will be no refunds for any reason. 

Insurance:

Luminous Developmental Services is considered an “out of network provider”. All evaluations are currently private-pay.

Cancellation Policy:

If you are unable to keep an appointment, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged a cancellation fee.
